1. A letter from you stating that your Board of Trustees/Directors has requested affiliation. Include all contact information such as your title, address, phone and fax numbers, email and web site (if applicable).
2. Copies of your ministerial credentials (must be earned credentials). In the event your Church/Center has a ministerial change, credentials for the new Minister must be approved by ANTN.
3. Copies of your church/center by-laws, incorporation verification, and Federal EIN verification.
4. A $500 application fee.
5. A short narrative of the history of your church/center, size of congregation and names of Practitioners.
6. Copies of flyers or material from classes or workshops you have presented at your church/center.
7. A history of your activity/leadership with previous churches/centers.
8. Three references related to your leadership activity within a church/center (include contact info for references).
